1. Purpose
The Apollo PCN High Intensity User (HIU) Service aims to identify patients who frequently attend urgent/emergency care, general practice and Community Services. This is also to provide proactive, coordinated, personalized support to address unmet biopsychosocial needs and reduce avoidable escalation.
2. Model of Care
The service is primary care (including Pharmacy) partnership with community, acute, voluntary sector, Local Authority.
3. Eligibility
Adults aged 18+ registered with an Apollo PCN practice, identified as high users of ED/GP urgent care, with evidence of unmet need. Identified through PHM.
4. Team Model
HIU Lead, GP Clinical Lead, Care Coordinator/Social Prescriber, with wider system partner input as required.
